How to Obtain Records

COVID-19 Latest News - Updated May 29, 2020

Effective May 29, 2020, the St. Charles County Division of Health Services’ Vital Records program has reopened to the public and follows normal operating hours (8:30 a.m. to 4:30 p.m., Mon-Fri). Individuals seeking Missouri birth or death records are encouraged to complete the required application prior to visiting our facility.

Please be aware that building restrictions are in place. Upon entrance to any County building, employees and the public are encouraged to wear cloth facial protection, and everyone is required to have a health screening, including temperature check, before entering. Those with a fever or who have other COVID-19 symptoms will not be permitted to enter the building.

Those who do not wish to enter the building may also complete the application below and mail it to the Vital Records program according to the instructions below or complete the process online through the VitalCheck link below.

Application Process (In-Person)

To obtain a copy of a birth or death certificate from the St. Charles County Department of Public Health (or any other local health department in Missouri), a person must present a document containing a photo identification - such as a driver’s license; passport; or school, work or military badge. If an applicant does not have a photo ID, they must present at least 2 alternate forms of identification. These documents must include the applicant’s name and the name of the company or organization issuing the document.

Application Process (By Mail)

Effective March 1, 2011, requests for vital records submitted by mail must first be notarized by a notary public. An application form (PDF) or a written request containing the required information must be completed and notarized before it is mailed (with payment) to the health department. Payment for mailed requests must be made by personal check or money order. Cash will not be accepted as payment for mailed applications

Application Process (Online)

As of March 1, 2012, you may order a certified copy of a birth or death record online using your credit card through our approved Internet ordering service, VitalCheckNetwork, Inc. An additional fee is charged by VitalCheck for using this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard®, or Visa®.

Other Internet vendors are not authorized by the St. Charles County Department of Public Health to issue these records and may charge additional fees without providing state-approved services. Neither the St. Charles County health department nor VItalChek have control over these services and fees offered through other Internet vendors, nor can we guarantee the confidentiality of any vital record data provided to vendors other than VitalChek.
